Repeated Security Violations at Your Facility: How to Find the Root Cause

One security violation is an incident. A string of them is a signal that something in the system is not working. When violations keep happening and nobody has the time or expertise to assess why, the risk grows with every repeat.

Common Violations

  • Tailgating and badge sharing at controlled entrances
  • Doors propped open or alarms disabled
  • Visitors and contractors left unescorted
  • Sensitive areas or information accessed without authorization

Step 1: Collect the Facts

Pull every violation report, access control log, and camera record available for the period. List each event by date, time, location, shift, and the people involved. Patterns often appear quickly: a single entrance, a specific shift change, or a contractor group.

Step 2: Look Past the Individual

Disciplining the last person caught rarely stops the pattern. Ask why the violation was easy or convenient:

  • Design: is the controlled door the only practical route to the break room or parking lot?
  • Procedure: is the escort requirement realistic for how many visitors arrive?
  • Training: do employees know the rules and why they matter?
  • Culture: do supervisors enforce the rules, or ignore them when production is busy?
  • Technology: are readers, alarms, and cameras working and monitored?

Step 3: Correct and Verify

Fix the cause, not just the symptom. Then check whether violations actually decline. A corrective action is only effective if the measured result improves.

Facilities handling export-controlled technical data under ITAR, classified information under the National Industrial Security Program, or high-risk chemicals face requirements beyond company policy.
